Small Area Rate Variatation

We have created maps showing high and low healthcare use communities and are working to understand the reasons why this happens. Some communities still have high or low concentrations of high healthcare use, even after we account for potential demographic causes. We want to learn why, and use what we learn to improve our approaches to working with communities and improve our care.

Integrating End of Life Care to Help People Stay in the Community
This knowledge synthesis grant focuses on understanding and supporting the complex needs of patients and their family caregivers who are dealing with multiple chronic conditions and/or life limiting illnesses.ÌýOur final product will provide a report on our findings along with recommendations for enhancing the implementation of innovative community-based palliative care models that address the complex needs of patients and family caregivers in the last year of life into bereavement.
